If it is your first time at Avon Chiropractic please select the Initial consult tab and arrive 15 min before your appointment time to fill out your paperwork. If you arrive and the chiropractor is with a patient please take the new patient paperwork from the front reception area.

If you only want soft tissue work done without adjustments please select the soft tissue only appointment
